How do I become a patient?

Please email me a picture of the front and back of your insurance -or- indicate you are interested in cash pay at: dev.brierley@foundations-wellness.com This email is HIPAA compliant. I will email you back an intake form to see if you would be a good fit.

Why wouldn’t I be a good fit? What then?

I only see patients via telehealth. Therefore, if I deem you are too high acuity I will provide you resources to find a provider in-person or refer you to a higher level of care.

Isn’t any care better than none?

No. Although, telehealth is an excellent mental health care delivery method it can be more difficult for me to monitor certain aspects of your care. In some cases, I will work closely with your PCP or therapist to fill this gap. Yet, this doesn’t cover all situations. If I am not delivering the correct level of care this can cause you, as the patient, more mental issues issues and damage.

What ages do you see?

18 years and up. I don’t see adolescents and children as I am a solo practitioner. I do not have the wrap-around resources I have found are necessary for the treatment of the whole family.

Do you do free 15-minute consultations?

I do not. This is because both my and your time are important. 15-minutes is not enough time for either of us to decide if we are a good fit. Generally, you have a lot to say! And I want to hear it! I want to hear your story and that takes time.

How do I make an appointment?

All current and potential clients please email me at: dev.brierley@foundations-wellness.com You cannot make an appointment on the portal or on this website. This is so appointments don’t get double booked.

Do you provide individual therapy?

No, I do not. I provide, “brief therapy.” My brief therapy is based on Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Motivational Interviewing (MI), and Mindfulness. I have an extensive list of both telehealth and in-person therapists I can refer you to. They are also on this website.

What do I do if I run out of medication and I am interested in being a new patient?

I cannot fill your medication without seeing your first.

Do you provide ADHD assessments?

Yes. I use the DIVA-5 evaluation tool which is validated for the diagnosis of ADHD. For more information please visit: https://www.divacenter.eu/diva-5/what-is-diva-5/ First, I do not prescribe stimulants or other controlled substances.
Secondly, not all other prescribers accept evaluations from ARNPs for the diagnosis of ADHD. If your goal is to treat symptoms with stimulants this could cause an issue. If this is a concern for you please look for a psychologist who performs these evaluations. There are quite a few on psychologytoday.com I do provide accommodation paperwork based on mental health evaluations for work/school for patients with ADHD.

Do you provide Autism Spectrum assessments?

No, I do not. I have not been trained to do so. Please look on psychologytoday.com for providers (generally psychologists) who perform this type of assessments.

Do you have a sliding scale?

No, I do not.

Why don’t you take Medicare/Medicaid/Molina/AppleCare?

My individual practice is just starting out. I am doing all my own billing/credentialing. State insurance requires more time than I have available to process these claims.

Why don’t you prescribe controlled substances?

First, the DEA is still in the process of deciding whether or not telehealth providers can continue to prescribe controlled substances without an initial/twice a year in-person evaluation. The DEA has created multiple deadlines and then pushed them. I do not feel it would be a benefit to my patients to start prescribing these medications and suddenly not be able to.
Secondly, managing controlled substances is time consuming. I have a chronic illness. I have able to manage it through keeping the complexity of the my practice as low as possible. I love to help people! But, I cannot do so, if I don’t take care of myself first.

Do you provide ESA letters?

Yes, I do. But first, we would need to discuss your unique situation and evaluate for the need.
